Re: riot issue
Riot guards or armed guards can go into rioting rooms without question. That's what they're paid for. Regular guards, on the other hand, didn't sign up for a full-blown riot. Soldiers can also do that, but unless you installed the Ultraviolence mod and called them in yourself, it's game over.
Re: riot issue
The cause of the riot? Well if i had to guess, either you had a bunch of gang members or a prisoner with the instigator trait in the kick off area. Either one would cause a mini riot that would raise the risk level through the roof.

turns out the riot was caused by a failing laundry room...seems to be cause by manually assigning which laundries handle which cell blocks..have tested this a few times now and i get the same result every time....if i let laundry auto assign its rooms it works fine but once i try to manually assign them it stops working entirely
Re: riot issue
But the Clothing need is supposed to not make the prisoners angry if not met. They just shouldn't feel well-treated if they have to wear dirty clothes, but not start a riot. (At least that's what the Alpha 11 changelog and video and the wiki say, and I think it's also in needs.txt, though that one contains incorrect information anyway, e.g. there's a chance of 0% for any prisoner to have the family need).

Wow...
Re: riot issue
I think this is a bug. I've also seen rioting prisoners unable to leave riot-controlled zones. It was as if the riot zone created an invisible wall, preventing anyone from leaving or entering.
