As much as I understand what you are saying here... I'd be more inclined to say that it is a player related issue and that you have not hired enough guards.

I have a prison setup where I changed the savefile to have 125 prisoners delivered in the morning. By noon they were all sorted into their cells and doing their routine. It's just a matter of having the necessary manpower.

Was the prisoner delivered to your prison, and his sentence was already completed?
