In general a doctor can perform the duties of a bin man without much retraining, however a bin man cannot generally perform the duties of a doctor without years of retraining. A doctor has thousands and thousands of dollars (or pounds) of education where as a bin man generally only has minimal education. Thus the doctor is worth more to society than a bin man. While both provide vital services one is a skilled job the other is one almost anyone can do. This is not to say one is better than the other, just one requires more education. However this is not really an issue of equity. Most bin men could be retrained to be doctors. However people are born with natural adeptness for particular skills. Not everyone can be retrained to be an artists of Picasso or Rembrandt's skill, nor can everyone be retrained to be politician of August or Alexander the Great's skill. Everyone has gifts that the general public does not. :)

Considering the NHS seems to have hygiene problems, that's obviously not true. The hopistals are in dire need of some people armed with a mop and bucket to stop the new infctions, not another doctor trying to treat them. And if the differnece between a doctor and a cleaner is thousands of pounds of education, society is saving money from having the cleaners instead.

The fact that the NHS is run by people who can't staff their facilities properly does not make my point incorrect. Any doctor could clean the floors, he just won't because it doesn't pay very well. :)

Because no one wants to do it, binmen and cleaners can be seen to quite valuable, as they are the only ones who'd even want to do their job.

Like I said their job is equally valuble, however as anyone can do it they themself are not as valuble to society. The pay issue is a reflaction of how much society values the job. If people will not work for that wage the wage will increase. Society's pay rates are balanced to make sure the right number of people can be found in each task, thus there should always be just enough people willing to work at a certain job for the given pay. That has nothing to do with the actual value of that worker to society, that is what the wage is. :)

I take it you don't count Hollywood actors, "musicians", etc as a part of society. The number of them, and the money they make, is ever increasing, and the number who want to be them is truely enormous.

I do count them. They are a very neccessary part of society and society pays them what it collectlively feels they are worth. In the past we killed people for entertainment now we just pay them millions, which I think is preferable. This is captialism for good or bad. :)
