Introversion

I have this massive Maximum Security prison. There are occasional fights but overall, it's controlled. However, the Danger Level won't go down. This has led to a handful of inmates dying because doctors can't treat them as apparently, it's too dangerous. This is also making it impossible to have a Shakedown without some cells not being able to be searched. I understand why this would be in the Supermax section but not in the regular Maximum Security section of my prison. I have added tonnes of guards and Armed Guards but still, nothing. Remove the armed guards. Their presence actually INcreases the danger level.
Hire a chief, then mouse over the dange level and its tooltip will show you what aggravates the inmates most.
