Prison a mess

Hello, i have a good running prison, but somehow when a prisoner attacks a guard it ALWAYS results in 2-3 dead people..

is there any way to prevent this?

What are that prisoner's traits? It sounds like you have a Legendary prisoner running around somewhere that's not secured against someone as deadly as they are.
