I've had quite a few prisoners walking around with serious injury, and not receiving medical treatment. It's been awhile since I've played, but If I'm not mistaken, aren't prisoners supposed to seek out medical treatment? Doctors should also go to solitary when a prisoner with injury is held there.
Also out of no where all my cooked stopped cooking. This is the 1st time I've ever had this happen. Anyone else had this problem?

It is entirely possible that through your building you created a path that the prisoners cannot legally (in terms of computer logic, not anything else, they are prisoners after all) use to get to the canteen, which the cooks see as "no one's coming to eat, better not cook".
Check your deployment zoning and ensure there isn't a zone block between prisoners and canteen (such as a staff only). **
Check any and all perimeter wall/fencing for holes if your prison is open and more than one building. Does not apply if the entire prison is contained within one huge building and/or double (or more) walled.
If you recently built any addition to or within the offending canteen(s), de-zone and then re-zone the canteen. I *think* it is a known issue that rooms don't always adjust properly to changes within the room (as in, if you add a wall to a room it still counts the wall as part of the room when in reality it is a wall and not a room). Having said that, "storage" rooms (at least) will adjust in A33 when a new foundation splits the room from outdoor only to indoor/outdoor, so there's that.
Do the same as the above with the kitchen as well. Shouldn't hurt anything.
If you just made new cells, ensure that they are part of a cell block (rooms inside a building with connecting hallway) and not free standing (individual buildings/rooms or a connected building with each cell "room" opening directly to the outside, e.g., no hallway). Same with solitary "rooms". **
Food trays are sometimes an issue, but shouldn't be here. Make sure you have enough cooks to wash them all and enough funds to buy more if not.
Whether you use micromanagement & logistics or not, you should unlock it (if needed) then check the supply/demand for your offending canteen and what is connected to it. This should at least hint to the problem or at best blatantly show you where the issue is.

** Possible resolutions above marked with ** should also help resolve this issue.
Minor injuries they will just walk off, take a nap and heal overnight (sometimes more than one night) but anything else they should be going to the infirmary on their own, or if they are not able, guards or doctors should be seeking them out.
