Suicide's popped up in the suggestions a few times, but what about the threat of self-harm instead of actual harm? Think of it as a heightened version of a 'complaining' prisoner - an inmate is so frustrated he starts letting the guards and other inmates know he's going to harm himself at the first opportunity. Then it's up to the player to decide how to deal with this - put the inmate in a cell block with more regular checks, give them things to make them more comfortable, etc.
The interesting part is whether the inmate actually intends to act upon it or not. How can you tell if an inmate just wants more attention or is genuinely on the verge of self-harming?

How you react to it could change the behavior of other prisoners, if you give them better things other prisoners are going to threaten to do the same to live a bit better. On the other hand if you do nothing there's the risk that the prisoner could pull a knife and while leaving a cell open for a hopefully more stabl prisoner should have a severe devaluation of the prison and have a pyscholgical impact on other prisoners, especially his friends.
