Cooks not cooking!

When I hire a cook and place him in the kitchen he immediately runs out through staff doors until he is outside. He will then just wander around.
He will not return to the kitchen to cook so all my prisoners are starving. Firing and rehiring doesn't help. What gives?

Obvious questions first, have the room requirements been met? Is this your only cook? Is the room "zoned" as a kitchen properly? A screenshot, if you're able to post/link one, would also be helpful.

Make sure the room "zoned" as a kitchen properly and all requirements are met.
