Have a look at this. This thread suggests that the game track deaths and escapes just like the contraband screen. That's the best way to handle this that I've seen suggested.

Unless the situation changed since Alpha-19, I remember prisoners seeing staff doors as simply a weaker jail door. They can request access through the door so long as they're allowed access to the zone on the other side. To the non-guard staff members, it's a door they have a key to. A workaround is...

I had this happen once. Was never able to reproduce, but essentially saving and then reloading the save corrected the problem. Try exiting out of the game completely and then loading if no success. Let us know if you still have a problem. Try removing and rezoning (possibly moving) the delivery zone...
