Basically the prisoners can get PTSD from certain, uh, actions done to them by other prisoners... You know... But after the incident occurs the violated prisoner must take a visit to the doctor. The prisoner will now be very sporadic with their actions due to the PTSD, and the only way to cure it is...

Elder Scrolls Online did this with their game, they implemented the entire console UI onto the PC version, it works very well, imo. Besides the obvious requirement of using a keyboard to talk to another player.
