By entrance they need a door of some sort. Under objects you can find normal doors, staff doors and jail doors. Maybe it happens when you get caught and when your points get put in and all. Probably then a bunch of guards and staff get respawned back into the prison.

Not entirely sure what the question is... But if you want to stop intake then go the the clipboard in the bottom right and click on the intake tab. There you can adjust how many of what prisoners you want and if you click on the "none" button no prisoners will be taken in.

Oh I thought this might be caused by you being a med sec and not all prisons supporting this security. Maybe not but this happened to me also on a few random prisons.

Honestly so many firefighters show up and things don't burn fast enough to really do enough damage without you killing each wave of firefighters. So I think that it is actually ok but I really just see the mode as mess around in your prison instead of a difficult build up and survive game-mode.
