I had this problem just yesterday and when i reloaded my map it sorted itself out.....

Sounds like such a simple thing but have you moved all cookers and fridges away from the walls in case they are the wrong way around etc?

Warden authorised executions may not be realistic but adding time to an inmates sentence for fighting, smuggling etc is a possibility though as this can happen....prisoner x forfeits his right to parole due to fighting in the prison on multiple occasions....that sort of thing

Well it seems the problem solved itself..... I reloaded the game and much to my amazement the cooks had decided to do some work....

I think it was the threat of being sacked and replaced with vending machines that did it....

There must be something we are missing as I don't see many others with this issue...
