The only issue with this happening to prisoners is the penalties you accumulate against those prisoners who have addictions and instead of getting treated, go to the foundation class every day. Only to fail dozens of times ...

It seems to be a bug with the last update. The only thing you could do to avoid this is remove any type of punishment in the policy tab. At least until it gets fixed.
