I had this issue as well sometimes after extended periods of time not adding an entrance. What I found useful is if you have a foundation with an entrance but it won't complete use the foundation tool and Right Click to cancel the foundation area. Then Left Click on the already build foundation area...

Did you happen to access the Bureaucracy sheet? If you click on the exit button of the Bureaucracy you will click the Bangup option at the same time. I found this bug today.
