EntityNeedsLibrary WARNING

When I go through the debug.txt I notice several of these error messages:

EntityNeedsLibrary WARNING: Somebody tried to release a provider that was not captured

Would anybody have an idea on what could be causing this error? Unfortunately the error message is way too vague to be able to deal with it. It should tell WHO instead of 'somebody' and WHICH provider instead of 'a provider'.

Besides the error above I've also seen this one:

EntityNeedsLibrary WARNING: Somebody tried to grab a provider that was already being consumed

What's going on here?
