There really are a lot of false positives, which can be quite annoying.
It seems to be sufficient to not pay much attention to the background noise of single yellow flags but to go looking for tunnels when there are multiple flags in a small area.

I think, staff and working prisoners should flee burning kitchens. First, it's ridiculous to watch cooks cooking happily in a kitchen were a cooker just burst into flames, rapidly filling the whole room with thick smoke fraom burning oil. Yeah, potted plants would add a nice little something. Kind of like the tree, but for indoors.
Maybe some of the features to upgrade cells could also be interchangeable, like having a plant OR a window to gain an additional room grade level.

I employ similar strategies as recommended by others (Trixi, 5hifty, Castaway, Pogmothoin etc.) above and I also find tunneling not that much of a problem. I find tunnels all the time, but I always find them in time. Doing a shakedown every couple of days does wonders, doesn't upset prisoners much (...

I find it's a good strategy to order a Bangup as soon as you get the outbreak message, to limit prioner-to-prisoner contact and thereby disease propagation. In my experience doing a shakedown every 2 or 3 nights is enough to uncover any tunneling activity before prisoners get do dig far enough to make an escape. I don't think this is very tedious.
